the arrow shattered, it must of had some damage to it and if carbon has it will come apart

Could be too high power bow on low rated or "slightly" damaged shaft.
On release, the shaft would receive tremendous pushing energy from the string,
a nicked or damaged shaft may shatter, break and resulted to accident like that.

Could he have had a rest that allows short shafted arrows? The arrow tip is often behind the hand with these type setups.

Thats not a nice wound to be wearing.
Carbon fibre is a wonder product but it does have its limitations.
Im a rower and National Umpire here in NZ and Ive seen Carbon fibre oar shafts explode when put under load. On examination its usually a stress point thats given way, those stress points may only be a pinprick in size but they sure have a way of ruining your day.
